About Children's Community Health Plan (CCHP)
Children’s Community Health Plan (CCHP), an affiliate of Children’s Hospital of Wisconsin, is an HMO dedicated to providing access to the highest quality health care and services to our members.
Children’s Community Health Plan offers two products to our Wisconsin members:
- BadgerCare Plus - CCHP is offered in fifteen (15) counties in eastern Wisconsin, and covers more than 130,000 adults and children, making it the largest BadgerCare health plan in the state. With the combined resources of Children’s Hospital, Children’s Community Services, Children’s specialty and primary care clinics, and our community partners, we are committed to improving the medical and social well-being of our members and the community.
- Together with CCHP - In 2016, Children’s Community Health Plan introduced Together with CCHP - a health insurance plan for individuals and families. Each plan offers members access to high-quality health care from our network of providers in Kenosha, Milwaukee, Ozaukee, Racine, Washington, and Waukesha counties. Providers include Children’s Hospital, Columbia St. Mary’s, Froedtert & the Medical College of Wisconsin, United Hospital System and Wheaton Franciscan.
Children’s Community Health Plan is a member of the Association for Community Affiliated Plans (ACAP), which is a national trade association representing more than 56 nonprofit safety net health plans in 26 states.
