Prenatal and Newborn Care

Children's Community Health Plan has a free prenatal care coordination (PNCC) program - Healthy Mom, Healthy Baby - for Milwaukee County residents that helps pregnant women get the support and services needed to have a healthy pregnancy and a healthy baby. We provide this service in person, in your home or at a community location.

If you are not in Milwaukee County, we can provide this service to you by phone or refer you to an agency in your community.

How can prenatal care coordination help me?
When enrolled in PNCC, care coordinators help provide services that are provided throughout pregnancy and 60 days after you have your baby. The care coordinators will meet with you monthly or more if needed in your home, coffee shop or place of your choice.
