The Healthy Mom, Healthy Baby program has prenatal care coordinators (PNCC) that will help you plan for your baby, as well as support your family with other services such as child care coordination.
Meet our PNCC Team:
Rosie, BSN, RN
Rosie attained her master's degree in rehabilitation counseling and bachelors' degrees in psychology and nursing through the University of Wisconsin-Milwaukee. She is currently working on a family nurse practitioner master's degree through Concordia University. She owns a Supportive Home Care agency in the Milwaukee area that provides support to individuals with disabilities in supervised apartment settings. Rosie has also been involved in teaching community nursing clinical rotations for Concordia University and Marquette University. Her passion for community health extends to global outreach in Kenya, Africa and Nicaragua, where she has been leading medical mission trips since 2010.
Ana, Interpreter and Case Manager Outreach Coordinator
Graduated with a degree of interpreter at The Institute of Interpreters and Translators in Mexico City. When Ana moved to Milwaukee from Mexico City, she was excited to share her knowledge of the Hispanic culture and language with our members.
Heather MSW, APSW
Heather holds Master of Social Work from Marywood University and is an Advanced Practice Social Worker. Heather has been doing maternal and family home-based visiting work for over 10 years. She most enjoys home visiting because it is an opportunity to work with women and families in a place they are comfortable and allows for more quality interaction.
Kathryn MSW, APSW, CLC
Kathryn holds a Master of Social Work from UW-Milwaukee and is an Advanced Practice Social Worker. She is a Certified Lactation Counselor and has a background working with children and families. Kathryn’s favorite part of her job is the opportunity to meet and interact with women and families, while building relationships that make a positive difference in their lives.
Kristin RN, CLC
Kristin holds a Bachelor's degree in nursing from UW-Milwaukee and is a Certified Lactation Counselor. She has many years of experience in providing prenatal coordination services. Kristin’s favorite thing about her job is knowing she is planting seeds of knowledge that will help improve the lives of the women and families she serves.
Meghan BSW, CLC
Meghan holds a Bachelor’s degree in Social Work from UW-Milwaukee, and is a Certified Lactation Counselor. Meghan has a background working with children and families and has worked at Children’s Community Health Plan for the last 4 years. Meghan has a passion for making a difference in the lives of others.
Susan RN, BSN, CLC
Susan has a Bachelor’s of Science in Nursing and is a Certified Lactation Counselor. Before coming to CCHP she worked in Children’s NICU for 20 years, so she’s obviously crazy about babies! She is passionate about seeing families grow and learn and coming alongside new moms and dads to make the best start on their parenting. She serves on the Advisory council of African American Breastfeeding Network
TaNesha earned a Bachelor of Science in Social Work from UWM. Her main background is in providing social services to meet the needs for all children and families. TaNesha believes in advocating for children’s rights to prepare for a better future.
