Together with CCHP believes in forging long lasting relationships with our broker partners. We are committed to providing excellent training and support, and to making both your members and your interactions with our health plan positive.
We currently partner with four General Agents to appoint brokers to sell our products. Before completing your application for appointment, please choose a GA. For a list of our General Agency partners, please contact us at CCHP-BrokerSupport@chw.org. Once you have chosen your General Agent complete the following steps:
- Complete our online Appointment Application
- Upload the required documents to you application.
Copy of your Individual Marketplace Certificate of Completion
Copy of your Errors & Omissions Certificate of Liability Insurance
Copy of your Wisconsin Health Insurance License
Provide a copy of your signed Business Associate Agreement
- Broker Portal Registration: Once your application is approved, you will receive an email instructing you to register for your broker portal. This email should arrive within 24-72 hours of your application submission.
- Welcome Training Materials: You will also receive an email from our Broker Support team with product documents to help you be successful when having conversations with potential members about our plans.
All commissions are paid directly to the General Agent. For questions regarding direct deposit and/or a commission schedule, please reach out to the General Agent directly. If they are unable to assist, you may contact our broker support team at CCHP-BrokerSupport@chw.org
