At Children's Community Health Plan, we believe good health is the foundation of strong communities. The following are local resources available to not only our BadgerCare Plus and Together with CCHP members, but providers in the Children's Hospital Network.
Visit CCHP staff at the Social Development Commission
We are pleased to be partnering with the Social Development Commission (SDC). The SDC is a community based organization that’s in your neighborhood offering programs and services to support employment, education, finance, health and wellness. You can learn more about the SDC by visiting their website here.
We have CCHP staff on-site at SDC to address concerns or questions you may have regarding your insurance, finding a doctor or dentist and how to receive help for any of your medical or behavioral health needs.
As a reminder, the SDC is located at 1730 W. North Avenue, Milwaukee, WI 53205
CCHP staff with be at the SDC Monday through Friday from 8:30 am to 430 pm. No appointment is needed to meet with us in-person. In the event that you would rather call us for support or to find a time to meet that works best for you please call us at 414-208-8437. We hope to meet you in-person at our new office!
