Silver Choice Plan

This plan is offered Off-Exchange only and includes all of the same ACA required benefits as our On-Exchange plans. This plan option is designed for those who may not qualify for financial assistance through the Federally Facilitated Marketplace.

CCHP on Call Nurse line
The out-of-pocket maximum is the sum of the deductible amount, prescription drug deductible amount (if applicable), copayment amount and coinsurance percentage of covered expenses, as shown in your Evidence of Coverage.
